Mouse problem

Lately, while playing game (Warcraft 3 - haven't tried another type of game yet), mouse would stop responding. My mouse is using a PS/2 to USB adapter, so I plug in the USB, mouse would work, but it keeps losing connection, then connects, then lose connection again...keeps doing it until I reboot. Is this a hardware or software related issue? It works fine otherwise, so I don't know. Maybe my mouse is failing. After all, I've been using it for around three years now.

It is a MS Optical Explorer 3.0 with the scrolling wheel, not the newer one with the tilt wheel.

Any hints will greatly be appreciated. Thanks in advance...
